Description

The Long Term Care Initiative (LTCI) has ensured that each Fraser Northwest patient in a long term care facility has a dedicated FP Most Responsible Physician (MRP). The Fraser Northwest Long Term Care community has coordinated efforts to support FPs in optimizing care for their patients in long term care through implementing these five best practices:

1. FP participation in care conferences 2. Proactive FP visits 3. Meaningful medication reviews 4. Completed documentation 5. On-call support network (24/7 availability and on-site attendance, when required)

FPs who work within the FNW LTCI are supported with a robust on-call network to provide after-hours coverage, a strong LTC medical advisory committee for peer and clinic support, and mentorship for those new to LTC practice.

There is a capacity for 5+ physicians for multiple facilities in New Westminster, Coquitlam, and Port Coquitlam. At this time, there are permanent vacancies at:

1. Hawthorne - Port Coquitlam

2. Cartier - Coquitlam

3. Royal City Manor - New Westminster

4. Nicola Lodge - Port Coquitlam

5. Kiwanis Care Centre - New Westminster

6. Madison Care Centre - Coquitlam

7. Belvedere Care Centre - Coquitlam

The positions include weekly onsite visits, depending on pandemic recommendations. EMR is care-home specific and the one most commonly used is Point Click Care.

Hours: Part-time Description Position: Part-time, long term Description: Medical care of adult patients in acute in-patient psychiatric unit Location: Royal Columbian Hospital, New Westminster Start Date: Flexible Join the Shared Care team at RCH and add variety to your practice! We are family doctors who look after medical issues on psychiatric inpatients. Our Department moved into a new facility in 2020, on the same campus. We had 32 beds and expanded to 72 beds including an older-adult unit. Our model is unique in BC. Shared Care physicians work along with psychiatrists, nurses, residents and medical students as part of a strong patient-centred team. The attending psychiatrist is the MRP. We assess newly admitted patients medically and follow them through their stay. Adult patients, all ages, with the usual variety of medical problems including co-existent chronic illnesses. Daily visits M-F, timing can be flexible. We deal with staff concerns, order and review investigations, and manage appropriately. Documentation is currently transitioning from paper to electronic charting. We are supported by full medical and surgical services within RCH, including lab, imaging and pharmacy. Call: Each FP takes calls regarding their own patients 0800-1700h daily. Evenings and weekends shared with other Shared Care physicians. Payment is sessional. The successful candidate(s) will be licensed to practice in British Columbia. (S)he should be comfortable with, or willing to learn about, psychiatric medications; and mindful of the interaction of medical and psychiatric health issues. This is an excellent complement to office practice; your new skills will help your office patients, adding to the comprehensive care we provide. It may suit IMGs as a facility component of practice. Description

Royal Columbian Hospital in New Westminster is looking to recruit a group of interested and suitable family physicians to provide the medical care to the surgical patients, initially on 4S/N as a pilot.

These physicians are responsible for providing care from 0700 – 1700hrs seven days a week. After 1700hrs, the surgical residents assess and deal with the medical problems, with the FP being the staff they would report and handover to. The physician group is providing a full scope of care from admission to discharge with a focus on quality assessments and comprehensive discharge/transfer out summaries. It is envisioned that a group of six or seven FPs could do a week at a time but the size of the group would be dependent on interest and availability.

This role has historically been funded by FFS and a call stipend. Part of the pilot is to assess how that funding model would continue to work and whether it would provide suitable compensation for this enhanced and comprehensive FP role.
